In 2013, parchment fragments were removed from the covers of four old printed volumes from the collection of Ondřej Horník that are now in the holdings of the Czech Museum of Music. By assembling the fragments, the museum has obtained seventeen incomplete pages of a Latin gradual probably dating from the period before 1420 that is in all likelihood of Bohemian origin (shelf mark: NM-ČMH AZ 89). The article provides a detailed description of the source, including references to the Cantus ID Numbers of the individual chants. The text deals with several Czech rubrics in relation to the work of Jan Hus and Easter folk customs, and it reports on the discovery of a trope to the antiphon Cum Rex gloriae Christus. Attention is also paid to questions of provenience, specifically to the town Polná, where Ondřej Horník obtained the music with the parchment maculature at the beginning of the 20th Century.
